An earlier church dedicated to San Michele is documented in the medieval period and was likely located near the Rocca. The 17th-century church was built to meet the growing needs of the population and the town, with the establishment of a chapter of canons.

The current façade, designed by the Faenza architect Boschi, was entirely rebuilt after severe damage from the 1781 earthquake. A large bronze door, created by the sculptor Angelo Biancini (Castel Bolognese 1911–1988), provides access to the interior.

Inside the Collegiata, the Madonna delle Grazie is venerated, a devotion widespread in the Faenza area, depicted in a panel alongside Saint Dominic, traditionally attributed to a painter named Mingarelli of the Romagna area, circa 1450.

Other works of interest include the baptismal font bearing the Malatesta coat of arms, three 18th-century paintings by the Bolognese artist Ercole Gaetano Bertuzzi (The Communion of the Apostles, Saint George and Saint Rosa of Lima), and two wooden statues from 1750 by the brothers Nicola and Ottaviano Toselli.

Previously, the church also housed the large altarpiece by Guercino, Saint Francis of Assisi and Saint Louis of France, now transferred to the Museo Ugonia, and The Adoration of the Magi by Marco Palmezzano, temporarily relocated to the Diocesan Museum of Faenza.
